NPC Advisory No. 2020 – 03 – A

SUBJECT: AMENDING NPC ADVISORY NO. 2020 – 03 ON THE GUIDELINES FOR WORKPLACES AND ESTABLISHMENTS PROCESSING PERSONAL DATA FOR COVID-19 RESPONSE
SECTION 1. This Advisory aims to amend Section 4 (A) of NPC Advisory No. 2020 – 03 pertaining to the collection of information by workplaces and establishments as required for COVID-19 prevention and control based on existing government issuances.
SECTION 2. Section 4 (A) of NPC Advisory No. 2020 – 03 is hereby amended to read as follows:
“SECTION 4. Ensuring fairness and lawfulness in processing. – xxx xxx xxx
- A. Establishments shall limit the collection of information to those purposes which are required for COVID-19 prevention and control based on existing government issuances: (1) Employee Health Declaration Form and (2) Client/Visitor Contact Tracing Form; xxx xxx xxx.”
SECTION 3. Repealing Clause. Any reference to the Department of Trade and Industry (DTI) Customer Information and Health Checklist is hereby removed. The pertinent provisions of NPC Advisory No. 2020 – 03 referring to the above DTI form are hereby repealed, amended, or modified.
